How to apply for disability abroad - instructions from the Ministry of Health

Since 2025, instead of medical and social expertise, a digitalized system for assessing a person's daily functioning based on multidisciplinary hospitals has been launched. In this regard, the Ministry of Health reportedthat the reform of the medical and social expertise system in Ukraine provides for the availability of services for citizens, including abroad.

For citizens of Ukraine who are in states with which intergovernmental agreements on social security have been concluded (Poland, Czech Republic, Slovakia, Bulgaria, Spain, Portugal), the following mechanism is provided:

  • Transfer of medical forms: the authorized body of the country of residence of the citizen of Ukraine transfers medical documents to the Pension Fund of Ukraine.
  • Document processing in Ukraine: The Pension Fund of Ukraine sends medical forms to the Ministry of Health, where they are reviewed by expert teams of the Center for the Assessment of the Functional State of Individuals.
  • Document translation: The Center for the Assessment of the Functional State of Individuals provides translation of documents into Ukrainian.
  • Compliance with international standards: Medical records must comply with standards such as the International Classification of Diseases (ICD-10).
  • Evaluation format: The assessment may be conducted in person or using telemedicine, depending on the specific situation.

For citizens of Ukraine who are in countries with which intergovernmental agreements on social security have not been concluded, the assessment process remains available according to the standard procedure:

  1. Preparation of medical documents: medical documents should indicate the diagnosis using the International Classification of Diseases, Tenth Revision (ICD-10), the degree of functional impairment, and data from laboratory and functional examinations (discharges of inpatients, consultations, examinations, etc.).
  2. Document translation: documents must be translated into Ukrainian and the correctness of the translation must be certified. According to the Law of Ukraine "On Notaries", no consular fee is charged for notarial acts performed abroad by consular institutions of Ukraine to certify the correctness of the translation into Ukrainian of documents required for submission for evaluation.
  3. Transfer of documents: translated documents can be sent by mail or transmitted to the attending physician via electronic means of communication. Based on the received medical documents, the attending physician will be able to form a referral for an assessment.
  4. Evaluation decision: based on the documents received, an expert team in Ukraine will conduct an assessment of daily functioning, which may take place in absentia or using telemedicine.

The Ministry of Health emphasizes that medical documents issued in the Russian Federation or the Republic of Belarus are not accepted for consideration.
